Assisted Living Care

ASSISTED LIVING CARE

We are committed to providing comprehensive care to help the elderly stay as healthy and active as possible. We discuss testing, medication, and illness with each patient and their families. We also provide the resources necessary to educate on wellness, disease prevention, chronic illness management and rehabilitation. Some examples include urinary incontinence, dementia, arthritis, restless leg syndrome, deconditioning and unsteadiness.

Having onsite practitioner services in Assisted Living represents a transformation. When you apply home based primary care to assisted living, you get a dramatic effect, including fewer hospitalizations, and readmissions.  We are able to prevent (emergency department) visits and hospitalizations and that is important to our patients and their families.  We can address problems such as pain, or acute condition change, promptly and avoid ED visit.  Staff can reach out to us and we can determine appropriate actions on the phone.

The ability to see residents onsite not only can prevent trips to the hospital but can save time, trouble, and the expense of transporting patients to physician offices.  Families and staff don't have to worry about this and can focus on caring for residents and spending quality time with them.  We collaborate with other specialists, such as podiatrists, dentists, to enable as much onsite care as possible.

When it comes to taking care of your health and wellness, you need a healthcare provider who can take care of you from head to toe. Beyond treating primary care needs, Diley Medical group offers certain procedures and podiatry care in the senior living setting. Some commonly offered services include skin lesions, joint injections, ABI, wound debridement, toenail removal, cutting/debridement of fungal toenails, diabetic foot exams and cryotherapy. Bringing these services to you makes receiving quality care more quickly and without an added external appointment.

Having our staff onsite allows the facility to meet the increasingly comprehensive health care needs of the resident and enable them to age in place with as much dignity and independence as possible for as long as possible.
